Reissue of Lost or Damaged Passport

Before you proceed to the desired service, you may consider the following registrations, if applicable

THERE IS NO TATKAAL SERVICE FOR ISSUE/REISSUE OF PASSPORT.

The applicant has to be physically present in the Embassy of India, Oslo for submission of documents. Regarding appointment, an e-mail may please be sent to [email protected]

Documents to be submitted to the Embassy

  1. Online Passport Application Form- https://portal3.passportindia.gov.in/Online/index.html Please apply in reissue category NOT in fresh.
  1. Two (2) recent photograph (5 X 5 cm), one to be pasted in square box on first page of the application form and one to be attached.
  1. Copy of valid residence permit front and back sides. In case it has expired or going to expire,please also submit letter/email from UDI mentioning that application for renewal of Residence permit has been submitted.
  1. Original Police report for Lost Passport (It is mandatory to mention Passport number in the police report)/ Passport in original (in case of damaged)
  1. Annexure F /Written Affidavit stating reason for applying for a New Passport / Explanation for Lost passport/Damaged passport. Please visit link for Annexure F https://www.indianembassyoslo.gov.in/page/forms-download/
  1. Copy of first and last page of  lost/damaged Indian Passport  . First page (which contains Name, DOB and other details) and last page (which contains Parents details and address)
  2. Any pages in passport containing endorsements
  3. In case of damaged, please also submit original passport

Provide copies of at least two of the following supporting documents as ID proof

  • Aadhaar card
  • Election card Payment details:
  • Bank Passbook Photo ID
  • Indian driver's license
  • PAN card
  • Expired Indian passport

Additional copies requested in case Minor

  1. Annexure D/ Annexure C (Use Annexure D when declaration is made by both Parents.Use Annexure C in case one parent has not given consent for issue of passport) http://www.indianembassyoslo.gov.in/page/forms-download/
  1. First page and last page of father’s Indian passport (First page which contains Name, DOB and other details and last page which contains Parents details and address)
  1.  First page and last page of mother’s Indian passport (First page which contains Name, DOB and other details and last page which contains Parents details and address)
  1. Father's residence permit front and back side
  2. Mother's residence permit front and back side

    In case resident permit has expired or going to expire, please submit letter/email from UDI mentioning that application for renewal of Residence permit has been submitted.

Noteworthy Points:

Ø Passport printable address has to be Indian address.

Ø While filling up online application form. please do not put Norwegian address in Passport printable address, in the field of other address details, Norwegian address may be incorporated.

Ø Please sign in the rectangular space provided on the first and last page of the online application form.

Ø In case of minor, please sign/put left hand thumb impression of the applicant on the first page of online application in the rectangular space and either parent has to sign on the last page in the rectangular space provided.

Ø For change in name or appearance, kindly contact the Embassy

Ø The Embassy reserves the right to call for additional documents as considered necessary.
