Birth Certificate based on Indian passport

The following documents are required to apply for a birth certificate based on Indian passport:

  1. Current Indian passport in original

  2. Copy of current valid Indian passport – picture page and address page.

  3. Copy of Valid Residence permit front and back side

  4. 1 recent coloured photograph (5X5CM)

  5. Written application stating the purpose for which you are applying for the birth certificate (citizenship, etc.) and contact details.
